Chronic pancreatitis and bile reflux

The majority of patients with pancreatic problems suffer from heartburn. Sometimes this heartburn is stubborn and does not respond to common treatment with the gastric acid suppressors. Many individuals have the bitter taste in the mouth, belching, or regurgitation with the bitter taste or vomiting of bile. The common belief that heartburn is caused only by an acid reflux is not always right, especially in a case of chronic pancreatitis. In this situation, the reason for the heartburn might be a bile reflux, biliary reflux that also goes by a name of duodenogastric bile reflux.

Bile reflux is not a new condition. The surgeons frequently have found presence of bile in the stomach during surgery, for over a hundred years. Normally, the liver produces alkaline bile and pancreas manufactures very alkaline pancreatic juice filled with digestive enzymes. The alkalinity of these fluids means healthy digestion and proper movement of theses fluids.

Why does it happen? Let us explain.

Bile is an alkaline fluid with a gentle balance of water, bicarbonates, minerals, lecithin, cholesterol, bile pigments, and bile acids and bile salts. If bile is acidic, bile acids precipitate in it. It is known; bile acids are exceptionally “aggressive” chemical substances. They make the acidic bile so corroded and irritable. The acidic bile irritates and injures the surrounding tissues, bile ducts, gallbladder, and sphincter of Oddi causing spasms, inflammation, ulcers. When the aggressive acidic mix of bile and pancreatic juice gets duodenum, it can create the chaotic spasms, and throws it up in the stomach and even into an esophagus. So there is bile reflux.

In the acidic pancreas and bile– fictional stage, there are occasional heartburns after large, fatty dinner with alcohol. Lab tests are normal, in the gastroscopy can be seen slightly inflammation in the duodenum, and stomach. Typically, a patient gets the diagnosis-GERD and treatment with the stomach acid suppressors. The person continues acidic lifestyle and alcohol consumption; thus, his condition deteriorates.

Chronic Pancreatitis and Sphincter of Oddi

For simplicity, imagine there are two rivers.

If there is no food in the duodenum, a valve – the sphincter of Oddi remains closed, retaining the bile in the gallbladder and pancreatic juice in the pancreas. Spasm or blockage of this valve can cause bile and pancreatic juice to back up. Just remember, a tiny amount of bile in the pancreatic duct can cause serious trouble. If sphincter does not close tightly, the content of duodenum with bile and active pancreatic enzymes can reach and injure pancreas through the opening of sphincter of Oddi.

It can happen due to mechanical reasons such as inflammation, surgery that cut this muscle valve, slipping of the large gallbladder stones, high pressure inside the duodenum due to collecting of gas and fluids, etc., and/or lack of proper nervous and hormonal regulation of sphincter contraction due to gallbladder removal, medication, alcohol, drugs, poor diet, stress, etc.

If the spasm of the sphincter of Oddi is prevalent in the common bile duct, pain is felt mainly in the middle or right part of the upper abdomen. If the spasm involves the pancreatic duct, pain is located in the upper abdomen on the left or right side and radiates directly through the abdomen to the back.

Even the initial attack of pain in the upper stomach area can be a signal to the beginning of the Sphincter of Oddi Dysfunction that can start the deadly chain:

When the stomach acid is suppressed, however, bacteria, yeast, and parasites may travel through, thrive, and cause trouble.

Some of the invaders, which survive in the stomach, move into the duodenum – the beginning of the small intestine. When the acidic gastric content escapes into the duodenum, a flow of bile is released

from the gallbladder, while simultaneously the pancreatic gland releases the pancreatic juice. Here the semi-fluid digested food from the stomach is neutralized by pancreatic juice and bile, which are extremely alkaline in nature.

Pancreatic juice contains potent antimicrobial substances, which make the pancreatic juice sterile. The enzymes that are designed to digest the proteins also digest unwanted visitors such as microbes, yeast, viruses, and parasites. Antimicrobial action of pancreatic juice makes the environment inside the small intestine clear and unfriendly for microbes and yeast. On the other hand, antimicrobial action of pancreatic juice also has a self-defense mechanism by killing the unfriendly bacteria. This process prevents infections of the pancreas and bile system by restricting bacteria from traveling through the duodenum to the pancreatic duct and gallbladder. Antimicrobial activity of the pancreatic juice depends upon pH. Alkalinity increases it, but acidity diminishes the antimicrobial activity of the pancreatic juice what we can often see in the case of SIBO.

The stomach of a person with SIBO looks like a balloon because of continuous fermentation and gas production in the small intestine. Next, the small intestinal tube has three openings – three gates. Why are these three gates mentioned? Because of bacteria, yeast and parasites can enter the small intestine only through these three openings.

One gate is at the beginning of the small intestine (duodenum). Here is a muscle valve that is called the pyloric valve. Semi-digested food from the stomach goes throughout this opening into the duodenum. If pyloric valve opens early, the gastric acid does not have enough time to kill microorganism. Thus, harmful bacteria, yeasts, parasites enter the duodenum.

The second gate is the Sphincter of Oddi – the opening in the duodenum where the pancreatic juice and bile go in the small intestine. The sphincter of Oddi dysfunction and SIBO are typically occurred together in person with the pancreatic disorders.

The third gate is located at the very end of the small intestine (ileum). Here is a valve – the ileocecal valve that connects it to the first part of the large intestine (cecum). The ileocecal valve has two main functions. The first is to prevent the backflow of fecal contents with bacteria and yeasts from the colon to the small intestine. The first is to prevent the backflow of fecal mass with bacteria and yeasts from the colon to the small intestine. Next is to avoid the mass of the small intestine from passing into the large intestine prematurely.

Imagine the following picture: There is an “employer” (human body), and there is its “employee” (beneficial intestinal bacteria).
